When the system,
(1)No rules and regulations in the department
(2)No clear responsibilities definition
(3)No process specify the way of working
(4)No reiview/verification/auditing
(5)No efficient cooperation between teams
(6)No shcedule control
(7)No quality assurance
(8)No supervisal
(9)No achivement-driven motivation
(10)No decisions at the endless and useless meetings

The system starts to become chaotic…

Managers start to be busy with issuing their orders…
Team members start to get accustomed to waiting the orders…

If luckily, the system will move forward stumblingly.
But if the key person leave or is out of the office, what about the situation?

As the time pass, more and more problems well up…

Manager starts to extinguish fire like a busy and exhausted fire captain.
But the fire becomes more active!

The perfect mess starts to become a perfectly mess!
